Required:
-Ban reason you receive when you get kicked

Optional:
-Your player name
-Game name (BF3 or BF4 or BF3/BF4)
-PBGUID(s) of your player(s)
-EAGUID(s) of your player(s)
-BattlelogID(s)/PersonaID(s) of your player(s) http://battlelog.battlefield.com/bf4/soldier/playername/stats/personaid/pc/
-Date you first noticed that you are banned
-Tell the truth ;)

Things not to do:
-Insults
-Hide information about stuff you did that might caused the ban (Stats padding, glitching)

Every player that wants to ban appeal should also be aware of our decision to enforce Cross-Game Bans:

Reason why server admins banning this playername is here

SyntaxErr0rr was used by 2 different players, 1 of them was cheat suspect as his K/H is high. Defib kill doesnt register as hit as far as i know and i didnt find any cheats there

How phucked up and messed is this account with assessment and whos going to sort it out  ...
